All these things would read better as
so and so, a level 3 Hyperwarp Navigaor, has quit the crew. someone else, a level 3 (Crew) has died during a combat.

I drew the crew danger card on exploration and it said debug in the outcome description. Right next to rolling skill save. This could be on purpose? At the moment we are considering this debug information. We're not sure what we will or won't display in the final version yet.

This edit may be too nit picky, let me know if it is so I can calibrate the type of feedback I provide.
Original Patrol Result: "Our scanners discovered a lost ship hanging in orbit of the system. Whatever happened to it was some time ago. It went down with all hands, but we can still recover valuable salvage from its ruin."
Comment: If a ship in orbit 'went down', I'd expect it to be on the planet. (I get that water going ships are said to have gone down with all hands.)
Suggested edit: "...Whatever happened to it was some time ago. It imploded with all hands, but we can still

tenbsmith: as a practical matter, put in whatever changes you like. This format is preferred. s/what we have/what I want instead/ (why) No particular reason why is needed, though sometimes it makes things clearer. And sometimes more discussion is needed, of course. Most of the time fallen will just go for it. Sometimes he won't. He's not allowed to take offence that you didn't like things the first way (and I can guarantee that he doesn't) and you're not allowed to take offence if he doesn't want all your changes. Rarely, an edit sparks a discussion -- as this one seems to have done.

But "went down" in the nautical context presumably means it sunk (or at least got torn apart and is now driftwood). I agree with tenbsmith that in the game text above it conveys the sense that the ship "went down" onto the planet. This might just be a difference of perspective, but it may help to have a less ambiguous descriptor.

If you want to keep the nautical flavour, 'was lost with all hands'. Hmm. Hands. Maybe what we should be calling Crew Dogs. I'm rather partial to 'all hands perished when such and such suffered catastrophic hull rupture' myself, but that is just me.

I'm fine with whatever is decided. Here's another approach, that retains the nautical language, but removes the ambiguity of did it crash.
"Our scanners discovered a wrecked ship hanging in orbit of the system. Whatever happened to it was some time ago. It was lost with all hands, but we can still recover valuable salvage from its ruin."
