Actually, I think it does work. I think the problem is trickier.
If I set Instadie and then scroll back to the top of the page to click the Save Settings And Continue button, it works correctly. In such a character, if I click the PDA button on my PocketComp, it includes a notation under "NoteBook" saying "Death Mode: 3".
If I set Instadie and click the phone's "back" button, this does not work. It is still a Custom Difficulty character, but the PDA does not have "Death Mode: 3" and it does not produce instadeath.
In many situations, the phone's "back" button does work as a "continue" function, such as in a hotel. But in custom settings, it apparently resets the settings to default. I frequently use the phone's back button rather than the game's continue or back button, so this confused me for a while. Are you doing that too, Gravling?
In any case, at the very least, I think there should be a "Save Settings And Continue" button at the bottom of the Custom page. It is a bit of a pain to scroll all the way back to the top after setting Instadie. My preference would be to also have the phone's "Back" button work like "Save Settings And Continue", but I can see an argument for having it work like a "reset" function. If that's what you want, then it should reset the customizations to default, and show the settings _without_ continuing, allowing the player to continue customizing.
