I just did two successful harvests and explores each on Blastblink, which is a magma world of danger 23. I never got Intrepid Explorer I, either on the planet or when going to the Palace afterwards. What am I doing wrong?

Just did it 3 more times and finally got it. Do you have to do it without getting hurt or losing crew or something?

fryhtaning -- I don't know I will check tho and post them. I bet it is something sneaky like "roll more positive points than detractor effects" or some abstract junk I summarized as "success"

I have also encountered problems with intrepid explorer. At 26,19 26,37 and 14,24 I have explored repeatedly. On several occasions I won substantial loot while suffering only mild losses/damage (and at least once, no harm at all). Same results harvesting. I have not received the award on planet or when going to the hall of records later.
