I play ST RPG like a machine -- daily -- leveling up past 10 on multiple Captains. I play all difficulties (except Basic and Normal) and have never encountered a "sudden unexplained" anything, and certainly never death without encounter!
If anyone has feedback on this that is actionable (like, I can do something about it) or a report that contains details I would love to work on fixing this.
Personally, I assume this type of issue would be one that would come up often if it really happened.
But, none the less I take all reports (even anonymous ones without details) seriously.

Personally never had this happen. I suspect the person reporting this had a sudden, unexpected combat death. That does happen, and ST RPG is (pleasingly) unusual in that regard. Players might treat it as a bug. And might not pay much attention to the whys and wherefores.

What he said was "no encounter" so I think it was during a space travel. Something like this happened to me before. My captain was so experienced and wealthy that the game play got little boring. So I bought the biggest ship I could with hundreds of crew and went for alien hunt (after some alien rumors). Suddenly - I've died travelling through a red space. It was a shock for me, because I knew I had full health. Later I've analysed what might have happened and found a reason for this (like loosing health in a boarding attempts and fighting the mutiny)
Obviously he (and I) has encountered radiation storm with 1 health. The question is - why he has lost 9 health before encountering the storm? I'm sure there were plenty of reasons and he (and me too) simply thought that he has 10 health, while it was much less. Showing the health bar/number on the map, combat, exploring screen would help to avoid such situations. It would also be nice to flash in red a toast about critical health when reaching health 1.

jerry -- that does explain it -- a radiation storms and mutiny are ways to loose health in deep space. Thank you I really like your ideas and they are now on the list.
