A simple request: any time you are going to leave an area that you absolutely can not go back to, can we have a dialogue that both makes it clear you can not go back and then asks if you want to leave?
This might already be true most of the time (the farthest I have gotten is early E2), but it is definitely not true all of the time. The example that just hit me was in the Torrent Watershed on E1. I was simply exploring, but I got to the edge of the map and that automatically initiated a bit of dialogue about how far Kern's End was and then whisked me away to Kern's End. There were definitely unexplored portions of Torrent Watershed that I wanted to explore, but I had no way of going back. I also did not realize that going to that location was going to make me leave the map.

Sounds reasonable, these have been added piecemeal to the game so far. Recently added one at the end of the Smith's Workshop to avoid the same fate for unfortunate players. 1. End of Torrent Watershed I think that might be the first one in the game? If other people are suggesting locations here, be sure to mark with SPOILER as samopop has played is early E2.

I think the initial dungeons are cases too (although I am going off of memory and these never bothered me): 1) Red Hill Dungeons 2) Forgotten Waystation

samopop - on these two, I disagree. They are the kind that allow for a natural continuation of the story and are clearly indicated by the map (RH) or there is nothing to return to (Forgotten Waystation) that you don't have on the far side.

Agreed. The only one that I have had an actual issue about (that I have reached so far) is the Torrent Watershed one.

The nordhall troll I believe you cannot reenter after killing the troll even if you have not cleared all of the other spawns

The end of ep.1, when you leave Oskahold with unfinished optional quest "Restless spirits of Silver Cohort". I wasn't expecting to be unable to return… Now, I have no way to turn it in.
