Baron Giovanni asks you if you think he'd be a good baron. I chose No on my only game. Would choosing yes make any major difference? If yes, what?

Blitz - the difference takes a while to percolate. In fact, the major branch that it causes isn't visible until E4.

Yeah, now you've done it. You realize that by saying No, the Baron takes your rejection to heart, becomes an evil dictator, takes over the tri-state area by building lots of -inators, and ends up choosing to fight against a fedora-wearing platypus instead of dealing with you.
Oh wait ... wrong channel.

I said yes on old phone... so said no now that I'm replaying on new phone to see what was difference. Doofens... umm... I mean Barrett does seem to take your opinion rather personally doesn't he? I still have suspicions about bloodlace too... her helpful suggestions always seem to lead to traps. Even though it wasn't my first time through ep1, part of me still expected her to turn evil. Hope we get to kill her later.
