Simple question. My status screen says I'm rank 3 and it's before the date specified on the achievement screen, but I don't get the award. Has happened several times. What gives?

Awards are only claimed if the Captain survives long enough to reach a Faction Palace and enter the sub-menu called "Hall of Records."
Within the Hall of Records you will find a button called "Record Exploits" which will check the Captain's Rank and Score and grant any Awards and Unlocks.
