I've been playing on Crazy difficulty and it seems like my sails are getting damaged more often later in the game. It happens somewhat frequently in green zones as well. It wasn't like this at first but it started a century or two into the game.
I ditched my First Mate well before this. A mechanic helps somewhat I think. But I'll get a notice that she repaired damage but I still take one sail damage.
I'm not carrying artifacts or anything. I've changed ships a few times but I didn't notice any problems immediately after the purchase.
Is this just something that happens later in higher difficulty games? Or is there possibly something I did to cause more accidents?

Does pilot and/or pilot upgrades reduce accidents? I've been looking for a way to reduce them and I though wisdom and the first mate and mechanic were the only ways to do so. I'm wondering too if large amounts of sails increases accidents.

Game challenge rate increases with each turn. The higher the Game Difficulty, the faster the game challenge rate increases. Are you still using that strategy of not spending XP?

Not really. I've played through a few captains spending XP as I go in pursuit of other unlocks. I actually got Templar Battleforce over the holidays and was away from ST. Excellent job on Battleforce--so immersive and engaging. It's really brought the game world to life. But I returned to one of my old Hard captains to finish getting my loot ships and victory unlocks. He had an XP stockpile so I did go back to the unspent XP 'strategy.' Working so far. Just looted my hundredth ship... now just 635 more victories to go I actually logged on tonight to search for Alien Hive advice. They always retreat before I can attack... I gather though that at higher levels they'll attack me rather than run.
