If you go on a capture mission, and you are unfortunate enough to be killed on it, and then have an encounter right after you wake up -- and the encounter ends up in a battle, and you escape from the battle --
you get the 'job failed' message.
This also happens if you are targetted by a patrol, submit, and then have an encounter, which leads to a battle, and you escape -- same as before.
It may be that this happens the next time you escape from battle regardless -- by the time I consider myself skilled enough to take capture missions, I don't die very often, and always because I screwed up the combat, alas.
