I've seen this happen multiple times, but I finally figured out what is going on.
1) Get into combat. 2) Kill all remaining mobs. 3) Discover a chest or some other dialogue situation before ending the turn in the above fight. 4) The discovery will end combat if all mobs are dead. 5) I'm 90% sure that you will find this hasn't taken turns off of your buffs. 6) Get into combat again. 7) Discover that on your first turn you are missing AP/MP which is equal what you had spent in the last, interrupted, turn of previous combat.
It appears that the discovery of some object interrupts the end turn routine and you begin the next combat in the same state as you ended the last.

The workaround for this is exiting out of the game and returning. You will be out of combat and #5 you can be 100% sure of, no turns are lost on buffs. Happens quite a bit to me, I just look at it as a buff extension or saving a potion or 2 not having to rebuff.

Obviously matrim was aware of it. One player's bug is a pro player's exploit! The players who take the time to report it with details and step by step instructions are much appreciated.
