I have been trying to somewhat manage my negative reputation with factions I am not trying to advance, but I have still managed to get around -50 or so with KH, LV, and the Streets.
So I have two questions:
1) Should I care? Or should I just be okay with having a couple of enemy factions who will always hate me and not worry about hacking them off?
I understand that CK should ultimately be played however you want as a player, but I am curious in how others prefer to play it.
2) If I do want to recover my rep with these factions, what is the best way to accomplish this quickly without messing up my hard-earned Fennian and Mars rep?

steely - missions and paydata are good ways to earn back Rep, but most Rep gain comes with some Rep loss. Avoiding them for a while too helps, as Rep trends back toward 0 over time.

Thank you As I thought about it more, I think I have answered my own question as to whether it matters to have a high negative rep. Obviously, since rep is added to heat, if I don't cool things down with unfriendly factions, then it's going to get rough trying to operate in their zones.

Dont kill. Use non lethal weapons. Makes a big difference. And, dont always target the same places. And avoid a lot of the optional encounters that cause conflict, but do the disarm and save people ones.
