Inadvertently got this when I "bumped" into a refugee shuttle in Insane....so much for easy pickings grrrr ;D

When I was at 2 health i did try to get the award in the palaces. After 4 planets I lost my patient and gave up. Went to doctor and got the award ^_^

Sorry, again -- I was unclear. How ironic.
I entered a bug report that the trigger for "Excellent Patient" is not explained anywhere. Most awards are granted at the palace but a few are given in various other places.
