I've got a level 15 Navigator who's had Neverlost since either the beginning of the game or maybe level 2...and she's never gotten the bonus. I just did a calculation of the probability of that and...basically, it's impossible. It's working for at least one other crew member, but not this one. Any ideas?

Maybe they have 12 Nav Skill?

I can't look at her anymore, because I got a Game Over, but the last time I checked, her (unmodified) skill was 11.
Besides, I thought the bonus was limited to 12, not skill + bonus? If that's not right, the descriptions for those Traits are misleading. And they wouldn't seem terribly useful once characters get to higher levels.

The Trait will raise the bonus to +12, not Skill + Bonus.

That's what I thought, yes. Just to clarify, she never had any bonus at all, just an unmodified skill of 11.
I'll tell you if I see it again. Chances are something went wrong there, though--maybe a weird interaction with another Trait. But I never took a screenshot, and anyway she still had one Trait hidden.

Ok, please let me know. While the probability is low its still possible that you got levels without bonuses.

Even if I assume she got the trait at level 2 (though I think she had it at start), the probability of that happening on even given run is 1 / 5^13, or 8.192e-10 (0.0000000008192). So, yes, theoretically possible, but if I played 1,000 games to the point where everyone was level 15 (I haven't--not even close), and 5 crew members each time had Neverlost (that's high even for Alta Mesa, but it happens), the chance of seeing a level 15 character with Neverlost and no bonus would be 0.0000042. It's reasonably safe to safe something went wrong, but that doesn't mean it will be easy to reproduce.
