I have been looking everywhere but I can't seem to find a specific list of content that is restricted to specific difficulty settings

The only content that is hidden on lower difficulty levels are enemy Captains.
There are some Unlocks (listed under "Awards") that only occur when you achieve an Award at a certain difficulty or above.
