Captain health droppig? No reason?

I finished exploring a planet, and I left with 4/10 health. I went to a couple different systems before I found a doctor. When I hit up the doc, I had 2/10 health.

Do Captains lose health by void hazards, solar sail damage, or retreating/acknowledging other ships? I didn't enter a conflict, no shots were ever fired. Was I "bleeding out?"

Itq happened again. Down to 1 crew, 4 health. I encountered a few while trying to find a doctor. The first planet I went to (level 15 rank, star hero rep) the doc was greyed out. While trying to find another one I retreated/acknowledged some encounters and then was told I was dead.

I was staid of playing that captain for that reason.
